Do you want to send one or more instruments/accessories back to us due to a complaint or as a return?
No problem. Please complete the attached document. You will then receive a PDF file via e-mail containing all of the details you recorded, which you simply need to attach as a complaint and returns note when sending the product(s) back.
You can generate your complaint and returns note in three easy steps.
1. Record the device details.
In order for repairs and returns to be processed quickly, please precisely describe the fault or reason for return.
2. Please enter your contact details.
Please also give the name of a contact person for any queries.
3. Your decontamination declaration:
Please inform us of any cleaning, disinfection and sterilisation measures that you have already taken.
